Good Faith Estimate Notice

Under the law, health care providers are required to provide patients who are uninsured or who are not using insurance with an estimate of expected charges for medical services.

You have the right to receive a Good Faith Estimate explaining the expected cost of your psychiatric care.

At Threaded Path Psychiatry, Good Faith Estimates are available for patients who are self-pay, uninsured, or choosing not to use insurance benefits.

A Good Faith Estimate may include:

  • Initial psychiatric evaluation fees

  • Follow-up appointment fees

  • Estimated frequency of appointments

  • Other reasonably expected services related to care

Please note that psychiatric treatment needs can change over time, and actual services or frequency of visits may vary depending on clinical needs and patient preferences.

Current self-pay rates may include:

  • Initial psychiatric evaluation: $425

  • Follow-up appointments: typically $225–325 depending on visit length and complexity

You may request a Good Faith Estimate at any time before or during treatment.
